Athletic Forms

Any sports physicals presented to Dr. Wiseman for review and approval MUST be on the current Athletic History/Physical Form (Rev 3/10). Any exams not recorded on the appropriate form will be rejected until the correct form is completed. The Athletic History/Physical Form (Rev 3/10) is the only form approved by the New Jersey Department of Education and has now been in use for an entire school year. 

Sports physical forms must be filled out completely - if the physician does not fill out all information (Example: for vision 20/___R  20/ ___L or Blood Pressure __________) we must reject the physical form.  Please make sure all spaces on the physical forms are complete.
